Expert Healthcare Advocacy When You Need It Most

Support for Families Navigating Complex Care, Led by an RN and Certified Case Manager

What Is Healthcare Advocacy and Care Coordination?

  • Healthcare advocacy is professional clinical oversight that ensures your care is coordinated, your data is accurate, and your voice is heard. Most people get lost in the healthcare system because they don't speak the language or know how to navigate between multiple providers. As your advocate, I act as the expert bridge between you and your medical team—translating complex information, coordinating communication, and ensuring nothing falls through the cracks.

    • Clinical Translation: We don't just "help you understand"—we audit your treatment plans and test results to ensure you have the full clinical picture and can make informed decisions.

    • Discharge Strategy: We manage the high-risk transition from hospital to home, closing the gaps where medical errors and readmissions usually happen.

    • Specialist Liaison: We vet referrals and coordinate specialists so your doctors are actually talking to each other and working from the same plan.

    • System Accountability: We navigate the insurance and provider maze to ensure your care moves forward without administrative delays or denials.

Why Lighthouse Concierge Care Is Different

  • Unlike general patient advocates, I'm a Certified RN Case Manager with a decade of clinical experience. I don't just coordinate, I review your care plan, catch medication conflicts, and identify gaps that others miss.

  • We prevent crises before they happen. Most advocacy services respond to problems after they occur. We identify issues early and solve them before they escalate into costly ER visits or hospital readmissions.

  • You get one dedicated point of contact who manages everything: specialists, appointments, medications, test results, and care transitions. No more playing phone tag with six different providers or wondering if critical information fell through the cracks.

  • Healthcare decisions create stress and conflict within families. As a neutral professional, I help everyone stay in their roles: parents stay parents, partners stay partners, while ensuring care decisions move forward with clarity and confidence.

What Lighthouse Concierge Care Is Not

  • It's important to understand our role. Lighthouse Concierge Care is a Clinical Advocacy firm, not a Home Care agency.

    • Home Care Agencies provide caregivers for daily tasks like bathing, dressing, and meal preparation.

    • We provide Clinical Oversight. We are the strategists who manage the doctors, the insurance, the medications, and the care roadmap.

Note: While Sonia is a Registered Nurse (BSN, CCM), Lighthouse Concierge Care provides professional advocacy and navigation services. We do not provide hands-on nursing care, medication administration, or medical treatment. We work alongside your providers to ensure they are delivering the best care possible and that you understand every step of the process.

Why I Do This Work

When my mother faced a medical crisis while I was studying for my nursing boards, I experienced firsthand how overwhelming healthcare navigation can be—even with medical training. Our family struggled to coordinate care, interpret conflicting advice, and make decisions under pressure.

That experience shaped everything. It showed me that families don't just need information—they need someone with clinical expertise who can translate, coordinate, and advocate when the system feels impossible to navigate.

I founded Lighthouse Concierge Care to be that steady, expert presence for families facing their most challenging healthcare moments. My mission is to provide the clinical oversight and proactive coordination that prevents gaps, reduces stress, and keeps you focused on what matters most: your relationship with your loved one.

We appreciate your kind words...

"Sonia at Lighthouse is absolutely amazing! She helped me understand my mom’s diagnosis when I was feeling lost and gave us a clear plan for home care. Because of her guidance, my mom is doing great. I even reached out a second time when another family member’s health became a concern. Both times, Sonia was patient and professional, turning a stressful situation into a manageable one. I no longer stress about taking time away for trips because I know she’s just a phone call away. I feel much more confident navigating these health journeys now. Thank you so much Sonia!"

"I had just left the ER still sick, and my doctor couldn’t see me for another three months. I felt overwhelmed and unsure of what to do next. That’s when I found Lighthouse. Sonia listened, took over the coordination, and within hours, I had a same-day appointment with a specialist, something I never could have pulled off myself. She found a pharmacy with the medication I needed, and even secured a telehealth visit when another specialist had a long waitlist. Thanks to her, I avoided another ER visit and saved time, money, and so much stress. If you’re stuck in the system and don’t know what to do next, Lighthouse is the partner you need."

“Sonia is an incredibly steady, straightforward, yet compassionate human and professional. She is a resourceful problem solver who knows how to make things happen. Her decade of experience shines through every conversation and yet she is a thoughtful listener - never assuming or pushy. Hiring Sonia provides a welcomed “middle man” for families in strained, emotional times. Thank you for what you do - I can’t think of a better small investment during a scary, overwhelming chapter of life.”
